Merchant cancellation/re-scheduling policy of 24 hours applies; voucher subject to forfeiture. No additional fees. Voucher available for use immediately after purchase.
*Customer must be a candidate for regular adult cleaning in the abscence of periodontal disease. If periodontal disease is present, treatment is avaiable for additional fees.
**Customer must be a candidate for Invisalign. If not a candidate for any of the offered services, customer will receive credit towards another service from merchant or a partial refund of the purchase price (25% of cost). If customer opts out of Invisalign treatment, $500 offer will be void and no refund for Invisalign coupon will be offered.
The amount paid for the voucher expires 1 year from the purchase date. The remaining promotional value of the voucher above what you paid for it is valid through the voucher expiration date, and will expire immediately after such date.
